    A productive cough is one of the most common symptoms of respiratory diseases. Coughing removes excess bronchial mucus (BM) from the airways. It should be noted that the definition of “productive cough” means the development of hyperproduction of BS, in which there is an increase in expectorated sputum.

    For example, the main clinical symptoms of chronic obstructive pulmonary disease (COPD) are cough with hard-to-clear sputum and shortness of breath, with productive cough appearing much earlier than shortness of breath. The effectiveness of cough and its intensity depend on the rheological properties of BS - viscosity, elasticity and adhesiveness.

    Ineffective cough (lack of expectoration of sputum) is often a consequence of significantly increased viscosity of BS.

    Exposure to air pollutants, primarily tobacco smoke, causes the development of inflammation of the mucous membrane of the respiratory tract, accompanied by its restructuring (in particular, a significant increase in the number of goblet cells and the area of ​​their distribution up to the terminal bronchioles) and hypersecretion of viscous BS. Cigarette smoke, as well as other air pollutants, cause oxidative stress, which inactivates various antiproteases, including a-antitrypsin.

    The secretion-forming elements of the bronchial mucosa begin to produce large amounts of sticky, viscous mucus. An increase in the content of neutral mucoproteins leads to an increase in the gel fraction in the bronchial secretion and, accordingly, an increase in its viscoelastic properties and adhesiveness (deterioration of the rheological properties of BS).

    Deterioration of the rheology of bronchial mucus (BM), the development of oxidative stress, and the direct effect of aeropollutants on the ciliated cells of the mucous membrane (metaplasia of epithelial cells) disrupt the effectiveness of mucociliary clearance, blocking its cleansing function. Increasing the viscosity of mucus and slowing the speed of its movement contribute to fixation, deeper penetration and colonization of respiratory microorganisms into the thickness of the mucous membrane, further worsening mucociliary clearance.

    Hyperproduction of mucus in combination with mucociliary dysfunction as the disease progresses can disrupt the drainage function of the bronchi and affect bronchial patency.

    With an increase in the viscosity of bronchial secretions, its speed of movement slows down (by 10-55%) or stops altogether, and at night the viscosity of bronchial secretions in patients with COPD is significantly higher than during the day.

    That is why patients so often complain of a severe cough in the morning upon waking up.

    Therapy

    Thus, in the treatment of productive cough (including in COPD), drugs are needed that have mucolytic, mucokinetic and mucoregulatory activity, facilitating the expectoration of pathologically altered BS, preventing mucostasis and improving mucociliary clearance.

    With the facilitation of the separation of BS in the bronchi, one of the factors causing bronchial obstruction is eliminated, and the likelihood of microbial colonization/respiratory tract infection (RTI) is also reduced.

    In addition, due to the development of oxidative stress, the need for the use of antioxidants, which have an anti-inflammatory effect on the mucous membrane and, indirectly, restore mucociliary clearance, becomes obvious.

    Criteria for the effectiveness of mucolytic therapy

    The effectiveness of mucolytic therapy (efficacy criteria according to Wettengel et al.) can be discussed by assessing its effect on:

    1. Well-being (improved quality of life). 2. Symptoms (reduction or disappearance of shortness of breath at rest or during exertion, reduction and relief of cough, change in the nature of sputum).

    3. Some indicators of external respiration function (characterizing bronchial patency), such as forced expiratory volume in 1 second, vital capacity function, Tiffno index, maximum volumetric velocities, and concentration of gases in the blood.

    All drugs that affect the rheology of BS and mucociliary clearance can be divided into 2 groups - direct and indirect action (see table). The peculiarities of the mechanisms of action of the most frequently and successfully used drugs - ambroxol, acetylcysteine ​​and carbocysteine ​​- require the attention of doctors and the correct selection of the drug in each specific case.

    Among the existing variety of drugs that alleviate expectoration, a group of synthetic mucolytic drugs should be noted. Ambroxol* occupies a special place in this group.

    Ambroxol is an active metabolite of bromhexine, a synthetic derivative of the alkaloid vasicine. Ambroxol was first synthesized in 1978 by Boehringer Ingelheim and is known in Russia under the name Lazolvan®.

    Ambroxol is superior to bromhexine in terms of the speed of onset of effect and clinical effectiveness, since it is a direct metabolite.

    Lazolvan® has the following important properties:

    — Mucolytic effect — Lazolvan® potentiates the activity of enzymes that contribute to the dilution of bronchial secretions by breaking down acidic mucopolysaccharides of sputum. - Mucokinetic effect - activates the movement of the cilia of the ciliated epithelium of the bronchi, restoring mucociliary clearance.

    — Mucoregulatory effect — normalizes the functions of the altered serous and mucosal glands of the bronchial mucosa and activates the production of the serous component of BS. This effect of Lazolvan® is especially important for patients with chronic lung diseases who experience hypertrophy of the bronchial glands with the formation of cysts and a decrease in the number of serous cells.

    A very important property of Lazolvan® is its ability to increase the amount of surfactant, both enhancing its synthesis in Clara cells and type II alveolar pneumocytes, and preventing its breakdown.

    Surfactant is a surfactant that prevents the alveoli from collapsing during exhalation by reducing surface tension at the liquid-air interface and protects the mucous membrane of the respiratory tract. Surfactant also helps remove foreign particles from the respiratory tract by participating in mucociliary clearance.

    It has bactericidal activity against gram-positive microorganisms and stimulates the function of macrophages, which contributes to the elimination of bacteria.

    Surfactant begins to be produced at the 20-24th week of embryonic development of the fetus, the surfactant system fully matures by the 35-36th week of intrauterine development and its intensive release occurs at the time of birth.

    Disruption of surfactant synthesis and its accelerated destruction can occur under various pathological conditions and therapeutic manipulations: hypoxia, inflammatory processes in the lungs, small bronchi and bronchioles, malignant neoplasms, exposure to tobacco smoke, infection of the respiratory tract (especially gram-negative flora, mycoplasma), diabetes mellitus, long-term (4-6 hours) fluorotane anesthesia, artificial ventilation, the need for long-term oxygen inhalations in high concentrations, the use of inhalation therapy using metered-dose aerosol inhalers, etc.

    Based on these provisions, it becomes clear that the use of Lazolvan® is useful in the treatment of patients with COPD, bronchial asthma, who are constantly receiving inhalation therapy, including long-term oxygen therapy.

    In patients in intensive care and resuscitation departments, including neurological ones, intensive and uniform sputum production is observed during the day against the background of inhalation or instillation of ambroxol in the DP.

    A number of clinical studies have shown that only two drugs have proven the ability to accelerate the maturation of fetal lung tissue when there is a threat of premature birth. These are glucocorticosteroids and Lazolvan®.

    Lazolvan® reduces the incidence of neonatal distress syndrome by 2-3 times, but is incomparably safer than glucocorticosteroids.

    It should be noted that at the birth of a child with neonatal distress syndrome, Lazolvan® can be prescribed to him from the first hours of life or to a nursing mother (ambroxol penetrates the placenta and into breast milk).

    The ability to synthesize surfactant is also potentially useful in the treatment of patients with acute exudative otitis media. It has been noted that the phospholipids lining the surface of the mucous membrane of the Eustachian tube are similar in structure to pulmonary surfactant.

    The effectiveness of Lazolvan® in the treatment of this disease was proven in a multicenter, double-blind, placebo-controlled study involving 435 adults and children (Mira et al., 1997).

    The effectiveness of therapy is due not only to the effect of Lazolvan® on surfactant synthesis, but also to its mucolytic effect.

    An important place in the treatment of respiratory infections (pneumonia, exacerbation of COPD, etc.) belongs to antibacterial therapy. However, the prescription of etiotropic therapy does not always have the necessary immediate effect. Antibiotics significantly increase the viscosity of BS due to the lysis of microbial bodies, leukocytes and DNA release. The excessive viscosity of BS makes it difficult for antibiotics to penetrate into it.

    Research

    Several double-blind, placebo-controlled studies have shown that Lazolvan® leads to an increase in the rate of passive diffusion of antibiotics from the blood plasma into the lung tissue without changing the concentrations of antibiotics in the blood plasma.

    Combination therapy of antibiotics and Lazolvan® in patients with various infectious pathologies of the respiratory system led to a statistically significant increase in the concentration of b-lactam antibiotics and macrolides in the bronchial secretions, a decrease in the severity of cough and an improvement in sputum discharge compared to patients receiving only antibiotics.
