The coming cold snap and rain may provoke a wave of colds, from which many want to protect themselves, and in the heat, doctors warn against the risk of getting sick, for example, by eating ice cream or excessively abusing the use of air conditioners.

Therefore, at the request of readers, we are reviewing Isoprinosin (also known under the brands Groprinosin, Riboxin and Ribonosin), which is positioned as an immunostimulating and broad-spectrum antiviral drug.

From what, from what

Isoprinosine is based on molecules of inosine and dimepranolacedobene. These substances, according to manufacturers, should either act to stimulate the immune system, calling on our lymphocytes to work overtime, or interfere with the reproduction of viral particles that have taken over our cells.

The authors of a 1986 review in the journal Drugs suggested that the drug does not have an antiviral effect, but does have an immunostimulating effect. The mechanism of this action, according to the same review, is unknown, and most experimental and clinical trials are preliminary, and their design leaves much to be desired.

Over the years, the situation with the mechanism has not become clearer. Inhibition of the replication of a whole list of viruses has been shown in cells (for example, in a joint study by French and American specialists, as well as in numerous studies by Polish doctors).

But how does it happen? Does this effect transfer to the whole organism, and is the concentration sufficient not to poison it? How is the replication of viral DNA or RNA genome suppressed by “spare parts” for these molecules, but the replication of cellular hereditary material (sort of) is not? Considering that the cytotoxic effect was sometimes assessed simply visually, and the publications mentioned appeared in national journals and were not even translated into English, it is unknown.

• New information has been added on binding sites: for example, in an article by pharmacologists from the USA and Taiwan, published in the Journal of Biomedical Science, it was suggested that inosine can act on the benzodiazepine receptor, but this is unlikely to have anything to do with the antiviral effect.

Isoprinosine’s instructions state that the drug suppresses the activity of a number of viruses (preventing them from forcing cells to synthesize their RNA), restores the functions of lymphocytes during immunosuppression, forces some immune cells to divide and trains others to see the “target”. It is recommended to prescribe it for infection with viruses of the Herpesviridae family (the most dangerous and common of which we have already talked about, for measles, human papilloma and molluscum contagiosum (the latter is also a viral disease, the mollusk has nothing to do with it).

On the portal of the State Register of Medicines, most of the registered studies concern bioequivalence (comparison of dosages at which the substance is equal in action) of different forms of the drug, as well as Russian and foreign tablets. Post-registration studies of the effectiveness and safety of the product are still ongoing. One of them was completed in 2015, but the publication could not be found.

Isoprinosine is included in the ATC classification of the World Health Organization - however, as we have repeatedly found out, this does not say anything about the effectiveness of the drug, but only makes it clear that the WHO knows about its existence. In this classification it is listed as an antiviral drug.

You can find hundreds of studies on Isoprinosine in PubMed, but if you filter “clinical trials” and add “double-blind controlled” to your search, you will see less than 10 studies.

A double-blind, randomized, placebo-controlled method is a method of clinical drug research in which the subjects are not privy to important details of the study.

“Double blind” means that neither the subjects nor the experimenters know who is being treated with what, “randomized” means that the assignment to groups is random, and placebo is used to show that the effect of the drug is not based on self-hypnosis and that This medicine helps better than a tablet without active ingredients.

This method prevents subjective distortion of the results. Sometimes the control group is given another drug with proven effectiveness, rather than a placebo, to show that the drug not only treats better than nothing, but is superior to its analogues.

A multicenter, double-blind, placebo-controlled study of 463 patients conducted by Hungarian researchers compared the rate of cure for influenza. Although the authors of the article report that participants in the drug group recovered faster, they also note that the difference was statistically insignificant.

If we talk about the prevention of infections in children, Isoprinosine could not stand out as a positive example here either.

The duration of ARVI in children also did not become shorter, and Isoprinosine had no effect on the number of days of symptoms (fever, cough, pharyngitis, runny nose).

Another study compares Isoprinosine with Acyclovir as a treatment for recurrent episodes of genital herpes. The first drug increased the time between episodes, but the work was carried out on only 31 patients, and no one has confirmed it since 1989.

Moreover, three years later, a publication was published about the results of another clinical trial, in which Acyclovir was the winner in a similar race. Isoprinosine was described as "clinically useless" at a dose of 500 mg twice daily.

But the work of Chinese, Japanese and German scientists on almost 300 patients showed that isoprinosine treats genital herpes and cold sores no better or worse than Acyclovir, completely confusing everyone.

Isoprinosine was tested on 17 patients against viral hepatitis C, but again it was not possible to prove that it was at least somewhat useful. Data from a study of 42 patients are consistent with this result. A disappointing conclusion awaits those who read the article about testing the drug against herpes zoster and against genital warts.

Finally, there is an encouraging article about successful treatment for viral papillomas (55 participants), but this result has not been confirmed by other studies since 1996.

(Not) wand - (not) lifesaver

But there are also less popular areas of use that relate to alternative therapy. So, taking inosine can increase uric acid levels in the blood, and since Parkinson's disease usually progresses more slowly in patients who have higher levels, inosine is now in phase 3 trials as a possible treatment for the disease.

With stroke, despite the effect on cells and experimental animals, there was also no progress. Perhaps because during an ischemic stroke, the delivery of nutrition to the affected area is disrupted, so that even beneficial substances do not reach their goal.

Isoprinosine was also tested as an alternative remedy for chronic fatigue, but this approach was also not successful: according to the authors of the review, the data was insufficient for conclusions.

In another study, Isoprinosine was tested in 24 patients as a treatment for rheumatoid arthritis, but there was no evidence of its benefit compared to a placebo group, but the level of urate (uric acid salts) in the blood of participants who received Isoprinosine increased. By the way, it’s worth thinking about the increase in the amount of urate in the blood - this side effect was mentioned in almost all articles about clinical trials.

Indicator.Ru concludes: the evidence is minimal, and it is not in favor of Isoprinosine

Isoprinosine is not approved by the US Food and Drug Administration (FDA).

This organization has many times refused manufacturers to register the drug and its direct analogues as a medicine for HIV patients, because patients can use it instead of, and not together with, specific antiretroviral therapy.

Although there is a clinical trial on this subject showing that the disease progresses less in the Isoprinosine group, the percentage of people whose HIV progressed to AIDS within two years is very low, so statistical bias could play a role.

Its mechanism of action at the molecular level is unknown, and all the effects observed in cells, tissues and rats are inconclusive at the clinical trial level. There are also no reviews on the Cochrane Collaboration website that would confirm the effectiveness of Isoprinosine (although there are no refutations either).

The organization's name comes from its founder, the 20th-century Scottish medical scientist Archibald Cochrane, who championed the need for evidence-based medicine and good clinical trials and wrote the book Efficiency and Effectiveness: Random Reflections on Health Care.

Medical scientists and pharmacists consider the Cochrane Database to be one of the most authoritative sources of such information: the publications included in it have been selected according to the standards of evidence-based medicine and report the results of randomized, double-blind, placebo-controlled clinical trials.

Thus, we did not find solid and convincing evidence of the effectiveness of the drug against anything. Studies in mice and cells may look promising, but what happens in patients is still a mystery. Although some clinical trials indicate that the number of T lymphocytes may indeed increase, there is no evidence that changing their number affects resistance to viruses (and to many at once, as would be the case with a nonspecific antiviral drug). However, even if this method works, it’s up to you to decide whether you should strive for such an unpredictable result: an overly trained immune system, like raging operatives, will “bind” innocent cells, which can provoke an autoimmune disease.

Although the drug is largely composed of a component found in human cells, it causes an increase in uric acid levels in the body. Unfortunately, this can not only slow the progress of Parkinson's disease, but also cause gout attacks, so if you have joint problems (and your diet includes a lot of animal products), caution is in order.
