Attachment of a patient to any drug dispensary is accompanied by his registration in a special database.
It’s probably not difficult to guess who is registered in the drug treatment department – these are people suffering from alcoholism, drug addicts, all those who cannot cope with the problem on their own.
This means that from the moment of registration, the patient must make timely visits to a narcologist.
Data about people registered at a narcological clinic are entered into the register by a narcologist. But this requires the written consent of the patient, otherwise the decision of the specialists can be challenged.
The law on drug registration applies to all persons without exception, but preventive surveillance is established exclusively for people at risk.
Driving a vehicle while intoxicated may serve as grounds for registering the driver with a narcologist. Any narcologist can tell you why they are registered at a drug dispensary; without being able to find out about it in the first person, a person searches for information on the Internet.
Detailed information about the rules for registering with a narcologist can be found in this review.
What reasons may serve as a reason for registration at a drug dispensary?
There must be certain reasons for registering a person with drug treatment. There are a sufficient number of drugs that have a composition similar to narcotic substances, these are painkillers for severe patients. Of course, you won’t be registered with a narcologist for taking a pill. The following factors may be good reasons for registration at a drug treatment clinic:
- driving a vehicle while intoxicated;
- alcohol abuse;
- use of prohibited substances, including single use.
Timely registration of alcoholics and drug addicts in the drug dispensary is a necessary measure. People who may pose a danger to society and those who are unable to cope with their addiction alone are subject to registration. This is done in order to provide the necessary treatment and support to the patient; often the initiators of contacting the clinic are relatives of dependent persons.
Procedure for registering a patient with a narcologist
The procedure is carried out at the place of residence, registration lasts one year, and throughout the entire period the patient is under home supervision. In some cases, dependent persons are referred for medical examination. A referral to a drug treatment center can be issued by the appropriate authorities, for example, the traffic police; a court may oblige you to go to a drug treatment center.
Only a psychiatrist-narcologist can make a diagnosis for a patient, since complex and dubious cases often occur, which are decided by a special advisory commission.
The patient is prescribed appropriate treatment in accordance with the drug treatment regulations. Personal information is collected at the place of residence of the dependent person; the patient himself or his relatives must provide the patient’s personal documents.
It’s not difficult to guess how they are treated in narcology, since the medical examination itself implies isolation of the patient. The person is isolated from society for the entire period of treatment, forced psychological rehabilitation measures are taken, and drug treatment is provided.
Clinical examination is used only in complex and advanced cases, but patients who are undergoing treatment at home know what dynamic registration by a narcologist is.
Types of rehabilitation and terms
As mentioned above, observation can be carried out prophylactically; in more severe cases, medical examination of the patient is required. What are the prevention and intervention measures, and what is the required treatment period? More about this.
Types of accounting:
- Preventive registration with a narcologist lasts exactly one year and is applied to those patients who are not dependent on alcohol or drugs. This is exactly the case when the object came to the attention of narcologists for the first time. To remove a person from the register, a doctor’s written consent is required.
- Dispensary registration with a narcologist has a longer period. Clinical examination is used if a person has been diagnosed accordingly. For this purpose, special diagnostic studies are carried out. The period of registration with a narcologist in this case is three years, the decision on its expiration is made by a special commission.
- An anonymous examination by a narcologist is when the patient himself consults a doctor. In this case, the patient’s rights are not limited, and the treatment is anonymous. At the end of the rehabilitation period, deregistration will not be necessary. The only caveat is that in this case the patient’s request and private treatment with a narcologist will be paid.
Mental disorders and other problems of drug addicts
Treatment of dependent persons is a necessary and forced measure, which is social in nature and an extremely necessary step for the patient to return to normal life. Promptly taken measures can guarantee the safety of society, since the behavior of drug addicts is inadequate and can lead to irreversible consequences.
Children can see frequent mood swings while on drugs, inappropriate behavior and other unpleasant things, which is why drug users are sent for compulsory treatment. How to recognize a drug addict? The following signs may be a reason to consult a doctor:
- physical exhaustion;
- the person’s appearance has changed (constantly red eyes, unhealthy skin, injection marks on the patient’s body);
- changes occur in the psyche, uncontrolled aggression appears;
- the addict experiences drowsiness and increasing apathy towards life;
- the drug addict's speech becomes slow and his movements uncoordinated.
While under the influence of drugs, a person can commit many illegal actions. Psychiatrists-narcologists write entire works about what addicted people see and experience under the influence of drugs, how the personality of drug addicts changes, how drug addicts sleep under the influence of narcotic substances.
Negative factors
Dependent alcoholics and drug addicts who are registered are deprived of their rights, isolated from society and subject to compulsory treatment. The status of a drug treatment patient implies a large number of restrictions associated with the freedom of movement of the dependent person.
What are the risks of registering with a drug treatment clinic, and what may be the consequences of drug registration:
- If the patient is registered with a narcologist, then he receives restrictions on work, for example, he cannot drive vehicles and work in positions associated with risk to others.
- Registration with a narcologist prohibits the carrying of weapons.
- The patient will receive refusals to hire if he committed an unlawful administrative action while he was registered. The restriction is valid for three years.
A registered patient cannot obtain a certificate from a narcologist for employment. The same rule applies when crossing the border and entering another country.
Positive points
However, not everything is as gloomy as it seems; a narcologist’s patient’s rights are limited only in severe and advanced cases. Let's say, if a person ends up in a drug treatment center due to drunkenness once, nothing will happen to him.
The maximum that threatens him is preventive accounting.
In severe cases, treatment cannot be avoided, but this has its advantages: a medical examination registered with a narcologist will allow you to quickly begin rehabilitation if an appropriate diagnosis is established.
While being treated at a dispensary, the patient will physically not be able to use the drug, and the moment of recovery will come much faster. In addition, the drug dispensary carries out special procedures that can get a person back on his feet and out of a critical condition.
Children, for example, students, are also tested by a narcologist. A certificate from a narcologist will be needed to obtain academic leave and subsequent rehabilitation of the child.
Treatment for any type of addiction in government institutions is provided free of charge. You can find out what tests are taken when deregistering a narcologist at the territorial offices.
Procedure for deregistration of a patient at a drug treatment clinic
To cancel the status of a dependent person, the patient must undergo a special commission from a narcologist. As a rule, the time frame for deregistration can be different: one person needs a year for treatment and rehabilitation, while another needs much more time. Discharge from a drug treatment clinic is not always accompanied by deregistration. The official dates are as follows:
- Alcohol abuse is punishable by one year of registration;
- Chronic alcohol use lasts for three years;
- In case of drug addiction, a sentence of five years is implied.
You can check whether a person is registered with the drug treatment department or not at the local territorial center. To deregister from a drug treatment clinic following a referral from the traffic police, you will need a whole package of documents, and you may even need the help of a lawyer.
In conclusion, it is worth noting that there are cases when a person signed with a narcologist while intoxicated, and does not remember why he was detained, and is also charged with some offense. In such cases, always seek the support of a lawyer, since any crime is fraught with unpleasant consequences.
— Tell your friends about the project It is very important to convey information about the dangers of drugs to children. To achieve this, psychologists and narcologists should come to schools and colleges to tell schoolchildren about the dangers of drugs. Modern society is taking active measures to combat drugs and their spread among young people, creating propaganda teams to combat drug addiction. Russia has been fighting drugs for a long time and exhaustingly. Almost every day distributors are arrested, supply schemes are revealed, and special centers for the treatment of drug addicts are opened. Children should be taught about drug addiction not only in educational institutions, but also in the family. Their influence on a child is many times greater than on adults, because we are talking about a weak, fragile organism.

How to get a certificate from a narcologist and a psychiatrist for the traffic police? Innovations in 2019
One of the responsibilities of every person is to maintain their own health. It is especially important to be healthy for those driving vehicles, because every driver is responsible not only for himself, but also for those around him.
In this regard, the state has established various requirements for drivers, one of which is the need to undergo mandatory examinations by specialist doctors. As part of this rule, a citizen applying for a driver's license must obtain the approval of a medical commission, which has recently included a narcologist and a psychiatrist.
Without a positive conclusion from these specialists, a medical certificate for the traffic police will not be issued. The procedure for undergoing medical examinations in drug treatment and psychoneurological dispensaries has a number of features.
Requirement of the Ministry of Health
The total time for passing a driver's medical examination has increased, because obtaining a certificate from a drug treatment center for the traffic police is a rather lengthy process. But without a permit from a psychiatrist and narcologist, you won’t be able to get a license in 2019.
The current order of the Ministry of Health allows some specialists to be trained by a general practitioner. For example, for categories A, B or M, a visit to an otolaryngologist is not necessary, and a therapist can send you for an examination to a neurologist if there are certain suspicions.
The new legislation provides for the possibility of obtaining the necessary document in the following medical institutions:
- State.
- Municipal.
- Private.
To provide medical examination services, the above institutions must have a valid license to provide such types of services. Most often, to obtain the necessary certificate, citizens go to the district clinic, which is located at the place of registration.
In 2019, the current form of the certificate in question is identified as 003-B/U. To undergo an examination by the necessary medical specialists, you must have the following list of documents with you:
- Passport.
- Driver's license, except for the initial acquisition of a license.
- For citizens liable for military service, a military ID is required.
- Two photographs for documents, size 3x4.
By contacting the registry of the selected medical institution, the citizen receives a medical card. It will list medical specialists whose certificates are required to obtain an opinion on the ability to drive a vehicle.
In most cases, all doctors are located directly in the building of the institution, except for two specialists: a narcologist and a psychiatrist. To obtain their conclusion, you must contact the neuropsychiatric and drug addiction clinics, respectively.
To undergo a neurologist and psychiatrist examination, it is necessary to contact a state or municipal institution. This is due to the fact that, starting from 2014, the seal of private clinics of this type is considered invalid. When obtaining a license in 2019, a certificate certified by a narcologist and psychiatrist from state dispensaries only is required - be careful.
Certificate of residence
As mentioned above, you can only get a certificate from a narcologist and psychiatrist at a state or municipal dispensary.
Current legislation establishes another mandatory requirement: where to get a certificate from a drug treatment clinic. In 2019, such a certificate can only be obtained from a medical institution at the citizen’s place of registration.
The registration status is not of fundamental importance; it can be temporary or permanent.
You can be examined by other specialists at the citizen’s place of residence. Examples of such situations:
- During a business trip, a citizen visited some specialists at a medical institution at his location. But he will be able to obtain the certificate required by the traffic police to obtain a driver’s license only after going through a psychiatrist and a narcologist and returning to the city where he is registered.
- Studen is studying in the capital, but he himself is from another city. He has the opportunity to go through all the specialists and receive a completed certificate in Moscow. The reason for this is his temporary registration, which gives him such an opportunity.
Similar data is entered into the citizen’s medical certificate (form N025/у). The driver submits the certificate received from the therapist to the registry office to make a corresponding entry in the journal; only after that, with all the necessary documents, can you contact the traffic police department.
Passing most medical specialists does not cause any particular difficulties for drivers, and not everyone knows how to get a certificate from a narcologist for the traffic police. This procedure has a number of features for which you need to prepare in advance.
There is a fee to see a narcologist, so the first step will be to pay for the provision of medical services. The citizen deposits money at the cash desk or registry of the institution, where he is given a receipt for payment. Some dispensaries accept payment through installed terminals.
The next step is to fill out an agreement for receiving medical services in triplicate. The driver must provide his personal information to the dispensary employee so that he can determine whether the citizen is registered in connection with the facts of alcohol or drug use. After completing all formalities, a citizen with the following documents:
- Reference;
- contract for services;
- medical card;
waiting to see a doctor. The narcologist conducts a visual examination of the patient for signs of use of illegal drugs.
If, as a result of the examination and conversation, suspicions arise about the possible use of narcotic substances by the driver, the doctor will refer the patient for tests (blood and urine).
Based on the results of the data obtained, the specialist can make a conclusion about the presence of symptoms that are contraindications to driving.
In most cases, an examination and conversation with the recipient of the certificate is enough for a narcologist to draw the appropriate conclusions. In most cases, a certificate from a narcologist for the traffic police is issued without testing. If no contraindications are identified, upon completion of the examination, the narcologist writes “Pass” on the card and certificate and certifies it with his personal signature and seal.
The procedure for undergoing an examination with a psychiatrist according to the scenario is similar to a visit to a narcologist:
- Payment for services to the institution's cash desk.
- Drawing up and signing a contract.
- Checking a citizen for his presence in the database of patients with mental disorders.
- Direct examination by a specialist.
There is one additional condition for applicants for a category C or D driver's license. To obtain a psychiatrist's opinion, it is necessary to present a document confirming the passage of an electroencephalography. This procedure tests the functioning of the brain at an electrical level.
For applicants for a category A or B driver's license, such a procedure is not required. It is only carried out if a referral is issued from a neurologist. If there are no violations after undergoing electroencephalography, a certificate from a psychiatrist is issued with a “Pass” conclusion.

Male under 55 | 3 |
Woman under 50 | 3 |

- Passport.
- If you have a driver's license, it will be required.
- Patient's medical record.
- A valid health insurance policy.
- For males, military ID.
If they are absent, the issuance of a certificate from a narcologist may be refused.
Get help quickly
If you need to obtain a certificate from a drug treatment clinic as soon as possible, you must follow the following procedure:
- Choose a medical facility that suits all conditions.
- Call the reception desk in advance to find out the appointment time for specialists and clarify the contents of the required package of documents.
- Prepare all certificates and documents required for passing the medical commission.
- At the appointed time, appear at the medical institution and pay the required amount for services.
- Collect your payment receipt.
- Get in line at the doctor's office.
- If required by the doctor, take the necessary tests on the same day.
- Receive a completed medical report.
When a certificate from a narcologist is not needed
It is not always necessary to go through a narcologist. The following are situations where a visit to a drug treatment clinic is not necessary:
- It is necessary to replace the license due to damage or wear and tear of the current documents.
- The replacement of the license is caused by a change in the driver’s personal data.
- The citizen himself expressed a desire to replace the rights before their expiration date.
- The driver's license has been stolen or lost.
If you are in doubt about whether you need to see a narcologist and a psychiatrist to obtain a license, it is better to contact the territorial traffic police department and clarify the information.

Passing a driver's examination with a narcologist and psychiatrist
Soon, traffic police officers will take away drivers’ licenses for certain medical reasons. Also, passing a medical commission will be required not only when passing the driving test, but also when the driver’s license expires. The innovations have already affected the procedure for passing a medical commission.
In 2016, some changes were made to the procedure for obtaining a medical certificate in form 089/083-u.
Previously, to obtain a medical certificate, you could go to any medical center, where, along with other specialists, you could see a narcologist and a psychiatrist.
Now, in order to undergo these specialists, you will need to contact the appropriate dispensary at your place of residence or place of registration.
A certificate from a psychoneurological clinic is now mandatory for the driver's commission.
Examination by a narcologist and psychiatrist
A driver's examination with a narcologist and a psychiatrist is carried out only in government health care institutions. Doctors bear full administrative responsibility in terms of medical examination, which, taking into account the adjustments, is now spelled out in the Code of Administrative Offenses of the Russian Federation.
To obtain a psychiatrist’s opinion, it is important to contact the psychoneurological dispensary at your place of residence or registration. When obtaining licenses of categories BE, CE, C, C1, D, DE, D1E, C1E, you must undergo an electroencephalogram of the brain. This procedure is carried out on the day of the specialist’s visit, also in a psychoneurological dispensary.

A drug test for a driver's license is carried out at a drug treatment clinic at the place of registration or at the driver's place of residence. To obtain a certificate, you must pass a rapid test for drug content in your urine.
Also, drivers at the drug treatment center must undergo a Schulte test during a medical examination. A certificate from a dispensary for a driver's license is required for all drivers. Drivers registered with a narcologist risk being left without a driver's license altogether.
They are unlikely to be able to obtain a certificate from a drug dispensary for a driver’s license.
Obtaining a license when the driver is already registered with a narcologist
Registering with a narcologist and driving a driver’s license are incompatible things. If a potential driver is registered with a narcologist, then problems may arise with learning his license. To obtain a driver's license, the driver will first need to be deregistered, and only then apply for the right to drive a car.
Refusal to test for drugs
Refusal of a medical examination for drugs entails liability in the form of a fine of 30-50 thousand or deprivation of rights. Read more in the article here.
This can only be done when a person registered with a narcologist has not taken drugs or alcohol for a long time.
To do this, you will need to undergo a mandatory medical examination, to which only those citizens who have already been deregistered by a narcologist are allowed. To do this, you need to undergo a medical commission; it is carried out on the basis of stable remission in the driver. The commission includes doctors from the medical institution where the citizen was registered.
An alternative to this option is the conclusion of a doctor who receives notes from institutions where the drug addict was observed. When a person is regularly registered and completely healthy, deregistration will occur within the prescribed time frame - after 5 years .
If it has been reported irregularly for five years, the commission will extend the period, and you will have to go through a narcologist for a driver’s license more than once.
Drivers who have ended up at least once in a drug dispensary because they were found using drugs in some cases do not suspect that they are registered with a drug dispensary and they find out about it when the time comes to get a license. Such drivers are denied a license. There are two options for resolving this situation: you can wait until the end of the period when they are deregistered; You can also agree with the attending physician to appoint an extraordinary medical commission, during which the driver will be removed from the register earlier. The situation is much worse for drivers who were subjected to a forced examination by a narcologist.
Compulsory medical examination
A compulsory medical examination for drugs must be recorded in a protocol drawn up by a traffic police inspector. The basis for this procedure is:
- the driver’s disagreement with the results of a drug test already carried out;
- refusal of medical examination;
- it is planned to initiate an administrative case against the driver, since there is every reason for this;
- traffic police officers have reason to believe that the driver is under the influence of drugs;
- employees have suspicions that the driver has committed a crime against road safety.
All grounds for conducting a medical examination are specified in the Code of Administrative Offenses of the Russian Federation. After determining the grounds, the traffic police inspector is required to draw up a protocol. The presence of two witnesses during this procedure is required. After drawing up a protocol and signing it by the traffic police inspector and the driver, the latter is sent for a medical certificate.
The basis for the examination is a protocol drawn up by an employee of the State Traffic Inspectorate. If the driver does not agree with the results of the examination or refuses it, then it is necessary for attesting witnesses to put their signatures in the protocol.
The driver must be accompanied by a traffic police inspector to the place where the procedure will be carried out. The examination is carried out in a medical organization that has a license to conduct medical activities with the obligatory indication of the relevant services and work. Sometimes examinations for drug intoxication are carried out in a specially equipped mobile unit.
The examination is carried out by a paramedic who has undergone special training at a drug treatment facility. If during a medical examination it turns out that the driver is not under the influence of drugs, the traffic police officer is obliged to take him to the place where he left his car.
The fact that the driver has passed a drug test is confirmed by a document that is drawn up in any form. Its validity period is three years. Based on the results, the paramedic draws up a conclusion: the state of intoxication has been established or not. If the fact of intoxication is established, the conclusion does not indicate the name of the substance that caused that intoxication.

How is an appointment with a narcologist?
Going to see a narcologist is a difficult step for many. Firstly, by doing so you acknowledge your problem, and this is not always easy. Secondly, not everyone knows exactly how the consultation takes place and therefore feels awkward.
We want to tell you how an appointment with a narcologist takes place in our clinic and how the diagnosis is carried out.
Visiting a drug treatment room - what to expect?
1. Collection of verbal information.
Reception is possible with or without an accompaniment. The doctor interviews relatives and the patient himself about complaints and concerns. It is very important to describe in detail the duration of the disease, complications, symptoms and not to omit information about attempts to recover in the past. For example, whether there were episodes of failure, how long they lasted and why the failure occurred.
In private with the patient, the type of drug used, dosage and effect are clarified. If the patient is reluctant to make contact or shows hostility, a psychotherapist is brought in to lower the wall and protective barrier to build constructive dialogue in the future.
2. Assessment of external condition.
During a calm conversation, the narcologist simultaneously analyzes the external manifestations of addiction, which is primarily reflected in behavior, coordination, conversation and the body’s unconscious reactions to questions.
The doctor examines the limbs to identify traces of injections on the skin, evaluates pallor or pronounced redness on them, and focuses on the smell (alcoholics exude a specific chemical, similar to acetone).
For greater accuracy, he performs palpation (for example, palpating the veins).
Additionally:
- Checks the reaction of the pupils to light. If a drug user is under the influence of psychoactive substances, inappropriately dilated or constricted pupils will be ignored.
- Reflex test with a neurological hammer. Harmful drugs reduce reflexes until they disappear.
- Measuring blood pressure and pulse, as they fluctuate when certain doping drugs are abused.
- Additional psychological tests are not excluded.
3. Laboratory examinations.
To move the dialogue towards a treatment strategy, the narcologist refers for tests to detect residual toxins and drugs in biological fluids.
Deviations from the norm will be noticeable even if their content is low (metabolites can be released within two weeks from the moment of taking the drug).
Depending on the results, he offers detoxification and a set of IV drips (glucose infusion, vitamin administration, injections with medications that improve metabolism).
If the need arises, they are sent for a general biochemical blood test, computed tomography, liver ultrasound, and electroencephalography.
4. Making a diagnosis and determining treatment.
Having visual data, knowing about the specifics of the clinical picture, he prescribes medicinal (coding for alcohol) or non-medicinal (psychotherapy, rehabilitation) ways to solve the problem, taking into account gender, age group, and personal make-up. For acceleration and better positive dynamics, they are used in combination.
To monitor the condition, it is recommended to visit the doctor after a set period of time, when changes appear or adjustments to the treatment plan are required. Sign up and find out more!

How to get examined by a narcologist and psychiatrist
Some activities are absolutely incompatible with the use of alcoholic beverages and/or drugs.
Therefore, when applying for a job or obtaining a driving license, a person must undergo a mandatory examination by a psychiatrist and a narcologist.
Also, such an examination may be required if a medical examination conclusion is required by a court decision, for registration or deregistration in a drug treatment or psychoneurological dispensary, and in other cases.
The examination includes a conversation with the patient and his examination. In some cases, a full examination is necessary, including testing for traces of drugs or alcohol. The laboratory can examine blood, urine and saliva, that is, the patient’s biological fluids.
How is the examination carried out to obtain a license certificate?
Most often you have to undergo an examination by a narcologist and a psychiatrist for a driver's certificate . It is necessary in the following cases:
- upon receipt of your first driving license. The doctor must make sure that the future driver is not dangerous either to others or to himself, and will not create a potential threat to traffic on the roads;
- when exchanging old expired rights for new documents;
- when returning driving documents that were previously confiscated by a court decision;
- if you have health problems, especially among professional drivers;
- when changing the category of driver's license;
- after the driver reaches the age of 50 years.
It must be taken into account that an examination by a specialized doctor is not a separate procedure, but part of the documentation that is necessary to obtain a medical report on a person’s fitness to drive a vehicle.
A narcologist is also a psychiatrist, so he asks the patient a number of questions that must be answered competently and clearly.
Based on the person’s answers, behavior, communication style and external examination, the doctor can draw conclusions about the adequacy of behavior and the potential presence of mental problems or toxic substances.
Stupid jokes, inappropriate answers, loud laughter and erratic behavior greatly reduce the chances of getting the necessary permission.
Typically, doctors limit themselves to a superficial examination of the sites where drugs are most frequently administered, most often the cubital veins. Just by a person’s behavior and appearance, one can suspect that he is using illegal drugs. In this case, he will have to undress to his underwear to conduct a full examination of the body, and if suspicious signs are identified, go for testing.
The cost of the examination and its location
According to the new legislation, the validity of a medical certificate is 1 year. It is issued at the place of residence exclusively by certified medical institutions of municipal, state or private ownership.
To obtain a driver's license, a certificate in the prescribed form 003-B/U is required.
Nonresidents can receive such a certificate from a narcologist only if they have lived in a new area for at least three years. This fact must be confirmed by a corresponding dated entry in the passport. If a student or a person who has recently arrived in another city needs a certificate, he has two options for obtaining the necessary document:
- go home and get a certificate in person;
- issue a notarized power of attorney for another person to obtain such a document for him.
Medical certificates for a driver's license are issued for a fee. Their prices vary depending on the form of ownership and the type of institution where they will be received. You can find out how much an examination by a narcologist and a psychiatrist costs when making an appointment with a doctor.

Procedure for deregistration from drug treatment
Drug registration is a registration that is carried out at a drug dispensary at your place of residence.
You will need to provide personal information about yourself:
- FULL NAME;
- place of residence;
- the place where you work or study.
You will also need to show your documents to prove your identity (this could be a passport or a birth certificate).
And after you have registered, you can already receive medical care at a drug treatment clinic that serves your area, and the course of such treatment will be completely free, according to the law.
Registration means that a person has been recognized as an alcohol or drug addict, and such a citizen is partially deprived of his rights. After all, drug registration is not a voluntary and not very pleasant measure, but it is very useful.
Registration:
- Accounting is control over a problem that is associated with alcohol or drugs, is of great importance in the country's strategy and is the prevention of alcoholism and drug addiction, treatment of addiction on a mental or physical level.
- Registration is treatment at the expense of the state in medical institutions, under the supervision of qualified doctors.
- Registration at a dispensary - exempts a person from military service on a legal basis. For some people and parents this is a big plus. But parents are different, they can deliberately send them to serve in the army so that their child stops dabbling with alcohol or drugs.
Drug registration: terms and types
Preventive accounting is based on the observation of patients whose dependence on alcohol or drugs has not yet been fully formed. For this species, the observation period is approximately one year. It is quite easy to register with such a register, and it is much faster to remove yourself from it than from a regular dispensary.
Dispensary registration - they diagnose a person who already has an established diagnosis of addiction. The terms here are already more serious, about 3 years for a person with alcohol addiction and up to 5 years for a person with drug problems. You can reduce the accounting time according to the rules, a meeting of the doctors’ commission.
Preventive accounting
If a person is admitted to the dispensary who has been found to be using narcotic substances, or is often found to be drunk, then such a person is placed on a preventive register. After all, with some narcotic substances, the effects on the body may not immediately appear, so such a patient is monitored. This is for if the patient has not yet been diagnosed.
The narcologist observes such a person for exactly 1 year. This means that under good circumstances, deregistration will occur exactly in a year, but if the tests show a negative result, then the patient will be transferred and you will be asked to take a more strict one.
Dispensary registration
People are placed on such a register when a diagnosis of alcoholism or drug addiction has already been fully formed. An alcoholic will be monitored for 3 years, while people who have drug problems will be monitored for about 5 years.
The patient undertakes to regularly visit the doctor and undergo periodic tests. And there is only one way to be removed from such a register quickly, this is only a prison sentence, soon more than 3 years.
Anonymous surveillance
This is for those people who themselves sought medical help. They want to cure their addiction without anyone knowing about their treatment or problem. In this case, all information about the patient will be stored only by the attending physician. The information will not be entered into various databases and will not be transferred to anyone.
The patient will have all the rights, he will not be limited in anything, for example, he will be able to drive a car. A person who is registered on an anonymous account is not required to go regularly, go for various tests or visit a doctor (when the patient wants, then he can come).
There is one drawback to this type of accounting - it is paid. The treatment period is already drawn up during the course of treatment.
Types of dispensaries
There are different types of dispensaries in Russia, such as:
- anti-tuberculosis;
- dermatovenerological;
- oncological;
- drug treatment;
- cardiological;
- trachomatous;
- orthopedic;
- endocrinological;
- neuropsychiatric;
- goitrous;
- medical and physical culture;
- ophthalmological.
Dispensary structure
In the structure of any dispensary, there are departments such as outpatient, inpatient and another department that deals with diagnostics (various treatment rooms, laboratories and much more).
For more efficient work of medical care for the population, which does not have a huge staff, where about 2-4 doctors work, they deliberately change them in the departments of ordinary district hospitals or, if in the city, then clinics.
Who makes the decisions—the drug treatment clinic?
Of course, all important questions and decisions are made by your doctor, a narcologist. He has no reason to keep you on the register any more than necessary. It will be better for the doctor if I remove you an order of magnitude earlier than necessary. After all, this will show that the doctor’s treatment is effective.
But the person who manages the entire department is responsible for registration at the dispensary. There is one point in his powers: he can assemble a commission, if there are reasons for this, which will decide the future fate of the patient. The commission can remove the patient from the register if he has success or simply shorten the period.
Negative aspects of dispensary registration
When a person is admitted, he is registered, and he is then officially considered sick and certain restrictions begin to apply to him, most of all these restrictions relate to human activities.
The person who has been registered is not allowed to:
- places where there is a high risk of fires or working with explosives;
- the person is unable to obtain a driver’s license or drive a car;
- It is not allowed to work as an electrician;
- work in the subway and with other types of trains.
The most important thing of any accounting is regular visits to your doctor. How often to go depends on how long your periods of sobriety are. Longer duration means fewer visits. But at the beginning of treatment, 6 months, you will have to visit the doctor at least once a month.
Some good private companies, when hiring, may ask for a certificate from a drug treatment clinic, and such a certificate can only be obtained when the patient is completely healthy and deregistered. A certificate may also be required when obtaining a visa.
If you are not registered, there is also no reason to get it quickly. Now it is mandatory for any citizen to undergo a medical examination to obtain this certificate. And if you do not have any problems with this, then it will be given to you in the near future.

Drug registration: useful points
- The first thing you should pay attention to is free treatment at the expense of the state.
- Your doctor's opinion can play a big role in court. But we must not forget that the patient can be protected only if he is registered and already assigned to the dispensary and the doctor who treats the patient can protect him
- And if, at the time you were registered, you were studying at some educational institution, you will be given an academic deferment, which will help you avoid expulsion
Consequences that are possible after taking into account
After you have undergone treatment and rehabilitation, you will be deregistered, unless, of course, you have given up alcohol or drugs. All the data that you were registered will not appear anywhere, and you will be given all the certificates, stating that you have never been addicted to anything.
How deregistration occurs
If you are detained by the police for drunk driving, then you need to contact a lawyer or undergo all the necessary treatment, taking into account the rules and procedures and the person will undergo drug registration within this time:
- unformed alcoholism - one year;
- alcoholism - 3 years;
- taking narcotic substances - 5 years.
The contract will be drawn up and the lawyer will ask for the following documents:
- a duplicate of the card of the person who was registered;
- forensic psychiatrists will draw up an application for deregistration after examining the facts of your card;
- a lawyer will defend you during the commission and in court;
- In rare cases, a lawyer may be called to explain all elements of the case.
It sometimes happens that the state asks you to provide a certificate from a drug dispensary to find out whether you are registered. And often everyone is sure that it will be quick, but it turns out that you cannot get it due to the fact that you are registered.
What should be done in such cases?
There are two solutions here:
- Complete the entire course of treatment according to the rules, take various tests, visit a hospital employee, for about a year. After which you go through a commission and receive the necessary certificate and are deregistered.
- Contact a law firm. Experts will check whether the procedure was legal; all actions of doctors are regulated by the legislation of the country. Most of it is in Chapter No. 704 dated September 12, 1988, where the main periods for observing a person are indicated. The lawyer will have to submit a representation to the judicial authorities, if later the doctor’s mistakes are revealed, it will be possible to legally be removed from the register.
There are often violations in this area, and for this it is necessary to raise the question of whether the registration was legal. And without a lawyer, deregistration is almost impossible, since ordinary people do not know the laws well.
List of professions where it is necessary to present a certificate from a drug treatment clinic
The Law of the Russian Federation of April 28, 1993, Chapter No. 377 “On assistance to citizens with psychiatric treatment” consists of the following professions:
- when working or producing benzene;
- a profession that has a relationship with drugs;
- work that relates to oil or other carbohydrates;
- pharmaceutics and pharmacology;
- working with explosives;
- work where weapons are used or carried;
- in rescue services;
- work as a fireman;
- driver of any type of transport;
- operation and maintenance of various trains.
In principle, most of these restrictions are justified. But it often happens that deprivation of some civil rights affects people who do not have an addiction, who have not taken drugs or alcohol for a long time but still remain registered because they do not have the opportunity to visit a doctor often.
And parents who are registered often encounter a problem such as deprivation or at least restriction of parental rights (Article No. 69 and No. 73 - Family Code of the Russian Federation).
It happens that such threats cannot always satisfy or meet the interests of the child, what can we say when a certificate from the clinic is considered the main argument or the only document for removing guardianship. And our court, in almost all such cases, makes a unanimous decision to remove custody of the child.
Accounting at the dispensary is governed by the following laws:
- General Order from the Ministry of Internal Affairs and also the Ministry of Health of the USSR dated March 20, 1988 No. 402/109 “Instructions have been approved on how to identify and register people who use narcotic or similar drugs for non-medical purposes, register and referral for treatment is compulsory.”

How to see a narcologist and psychiatrist to obtain a driver's license | Moscow auto lawyers
Before the changes adopted at the end of 2016 to the procedure for passing a medical examination to obtain a license, it was much easier for drivers to obtain a medical certificate.
Before the adoption of these changes, a candidate for a license could bypass everyone as a doctor in one clinic (private or public, at the driver’s choice), including a psychiatrist and narcologist.
Now, as many drivers are already aware, in order to obtain certificates from these doctors, you must visit a drug treatment and mental health clinic (and only state-owned; private clinics are not allowed) strictly at your place of registration.
Certificate No. 089/083-u has also undergone changes, the format has changed, it has become colored, and most importantly, there have been changes in the rules for filling it out and the type of information entered. The list of cases in which a medical examination is mandatory has also changed. Read more about changes to the medical certificate form in the corresponding article in this section.
Examination procedures in drug treatment and psychodispensaries
As mentioned above, after changes were made to the law on passing a medical examination to obtain a driver’s license, examinations by a psychiatrist and a narcologist are carried out exclusively in state dispensaries. The relevant doctors are personally responsible for the results of these examinations (administrative liability has been introduced for incorrect diagnosis).
Both before and after the changes, a narcologist and a psychiatrist are required doctors to undergo a medical examination for a driver’s license. You must pay for your appointment from your own funds. When going to a psychiatrist or narcologist, a man only needs to take his passport and military ID (or registration certificate), while women only take their passport.
So, an examination by a psychiatrist is carried out at the state psychodispensary at the place of registration. For some categories of rights (“B”, “C”, “D” and their subcategories), it is also necessary to do an EEG of the brain. It is done there and on the same day.
You need to see a narcologist at a drug dispensary, also exclusively at your place of registration.
In addition to a visual examination and questioning, a citizen must submit his urine for analysis to detect traces of drugs in the body, and also undergo the procedure of taking the “Shulge test.”
Since an examination by a narcologist is required to obtain a license, citizens registered at a drug dispensary will not be able to satisfactorily pass the examination, and therefore will not be able to obtain a license.
Is it possible to undergo a medical examination to obtain a license if a citizen is under supervision at a drug dispensary?
As everyone knows, citizens registered at a drug dispensary will not be able to obtain a license, since they simply will not pass the mandatory medical examination. In order for a person registered at a drug dispensary to have the opportunity to obtain a license, he must first be removed from this registration.
And in order to be deregistered, a citizen must abstain from using drugs and alcoholic beverages for a sufficiently long period of time. This must be confirmed by the narcologist who is seeing the citizen, and then by a special commission that decides whether or not to deregister the citizen based on the narcologist’s observations.
The commission consists of doctors from the dispensary where the citizen is being observed. To be able to collect a commission and deregister, a citizen must not use drugs or alcohol for 5 years in a row, and regularly check in with a doctor to record this period.
If there are breaks in the marks, the continuous period will not be counted and will be extended.
Those citizens who have at least once passed a positive medical examination for drugs or alcohol are already registered, although not everyone knows about it. And they are confronted with this fact when they need to get a medical certificate again (replacement of license, return after punishment, etc.).
These citizens will definitely not pass the medical examination, since they will not be given a certificate from the drug dispensary.
They have 2 options: either wait 5 years until they are deregistered (regularly checking in with a doctor), or ask for an urgent medical examination, according to the results of which the citizen can be deregistered early.
In what cases is a mandatory medical examination carried out?
There are cases when a driver is forcibly sent for a medical examination to determine the presence of alcohol or drugs in the body. In such a development of events, a protocol must be drawn up, where a note about compulsory medical examination is made. Typically, compulsory medical examination is sent in the following cases:
- If the driver categorically denies the results of previous alcohol and drug tests;
- If the driver does not want to undergo a medical examination for the presence of alcohol and drugs in the body;
- If the driver is at fault for the accident;
- There are all the signs that the driver is drunk;
If there is one of the above signs, the traffic police officer looks for 2 witnesses and, in their presence, draws up an act of referral for a compulsory medical examination.
One copy remains with the traffic police officer, one is given to the driver, after which it is delivered to a medical facility.
If the driver refuses a voluntary examination, a note to this effect is made in the report and certified by witnesses.
A traffic police officer must take him to the clinic or hospital where the driver will undergo a medical examination. This medical institution must have a certificate for this type of activity, as well as appropriate equipment and doctors. Also, the functions of a medical examination can be performed by a mobile medical center.
The medical examination procedure for the presence of alcohol or drugs is carried out by a doctor who has undergone appropriate training. In the event that a medical examination and relevant tests do not reveal any prohibited substances in the driver’s body, the traffic police inspector takes the driver to his car.
For any result of a medical examination, a protocol is drawn up where the results are recorded. This protocol is valid for 3 years. The conclusion after the medical examination is drawn up by the doctor who conducted it. If prohibited substances are found in the driver’s body, their name is not written in the report.

What diseases does a narcologist treat? Find out at DocDoc.ru
Narcology is a narrow branch of psychiatry that studies diseases associated with various pathological dependencies on various substances and processes (while psychiatry in general includes the treatment of any mental diseases).
A characteristic feature of the work of a narcologist is the patient’s obligatory desire to be cured, since in the absence of this condition, treatment will be ineffective. A narcologist works with a patient without his informed consent only if the latter is insane, or by a court decision.
It is also important for patients to know that private clinics provide treatment on an anonymous basis, strictly observing medical confidentiality.
What does a narcologist treat?
What narcologists treat can be described in one word – addiction. Narcology deals with diseases of the human brain, which are expressed in a persistent dependence on certain actions: taking drugs and alcohol, smoking tobacco, gambling and pathological attachment to the computer.
What diseases does a narcologist treat?
Narcologist’s patients are people who have been diagnosed with:
- alcoholism at any stage;
- addiction to hard drugs;
- drug addiction;
- pathological addiction to smoking;
- “gambler” syndrome (inability to independently refuse gambling);
- mental dependence on computer use.
How does a narcologist treat
Currently, narcologists use two main methods of treatment:
Psychological influence - coding in various ways (hypnosis, suggestion, etc.), group therapy, conversations. The method is effective if the patient has a persistent desire to get rid of addiction.
Treatment with medications is used in difficult cases when the body cannot cope on its own, serious changes have occurred in the psyche, or the patient insists on this method of treatment, considering it more effective.
How does an appointment with a narcologist work?
At the first appointment, the narcologist:
- assesses the general condition of the patient;
- determines the degree of deviation from normal worldview and lifestyle;
- prescribes tests (blood and urine, lung scans and liver ultrasound) necessary to identify specific pathologies of drug addicts, smokers and alcoholics;
- A program of outpatient or inpatient treatment is drawn up, including consultations with a therapist, gastroenterologist, cardiologist and neurologist.
